Great, thanks for testing. The build is green as well, so I've marked this ready for review. |

Brings in 11 upstream commits from 2026-10-04 to 2026-10-16: the icon cache fixes (use-after-free in the save thread, index checks), context menu commands back on the UI thread, the false (Empty) entry in custom submenus, Win+X and the hover timer, Setup restoring Explorer, and dark mode for Classic Explorer. Upstream Open-Shell#2558 fixes the same Explorer crash as our fix-settings-accessibility branch (stale accessibility servers of the settings tree after the DLL unloads), with the same calls in the same two OnDestroy handlers. The conflicts were only there, so they were resolved by taking upstream's ClearSettingsTreeAccessibility and dropping our DisableSettingsTreeAccessibility. Our column changes in SettingsUIHelper.h and SettingsUI.cpp merged untouched.

Summary
Release the MSAA dynamic-annotation servers owned by settings trees before their hosting DLL can be unloaded.
Root cause
The accessibility changes from #2518 register
CSettingsTreeAccServerinstances withIAccPropServices::SetHwndPropServer.Those registrations are not currently cleared when the settings dialogs are destroyed. After Open-Shell exits,
StartMenuDLLcan therefore be unloaded whileoleaccstill retains marshaled references to aCSettingsTreeAccServer. The dump in #2556 shows the later cleanup path trying to dispatch
CSettingsTreeAccServer::Release()through the oldStartMenuDLLaddress, which matches that lifetime mismatch.Changes
This does not change accessibility behavior while a settings dialog is alive; it only closes the lifetime of the dynamic annotations explicitly before teardown.
