pylcm now decides from the reads whether a shock carries an axis: one read only as `next_<name>` is drawn after the choice and integrated out. The transitory medical-cost shock is read only as `next_hcc_transitory`, so it is no state under any grid configuration and the `fold_hcc_transitory` knob goes.
