Runbook: GitHub App callbacks, Members permission and linking an existing install - #232

Step 1 now lists the dashboard's installation callback first (GitHub returns installs and updates to the first callback URL), turns on user authorization during installation and redirect on update, adds the Organization -> Members read permission the link's admin check needs, and the App logo. Step 4 explains that re-running a deploy after a failed startup probe applies nothing and how to roll a revision; step 5 shows how to link an App that is already installed. Updates
docs/infrastructure/gcp-cicd-setup.mdwith what turning on the GitHub App in staging (#231) taught us:/dashboard/api/github_installations/callbackgoes first in the App's callback URLs, because GitHub returns installs and updates to the first one. GitHub won't save a reorder, so an existing App needs its other URLs deleted and re-added in one save.GET /user/memberships/orgs/:org, which answers 403 without it (Projects ignore GitHub App installations; App setup gaps (Members permission, return to dashboard) activeagent#645).installation_id.Docs only.
