#1314 bounds cleanup frequency and deletion batches, but finding expired rows can still scan the whole table. This follow-up gives PostgresKvStore an indexed candidate lookup while retaining that housekeeping policy.
Store expiration keys computed with UTC calendar arithmetic in a partial expression index rather than a new column. PostgreSQL populates existing keys during the index build and maintains them for old writers too, avoiding triggers, a separate backfill, and coordinated application upgrades.
Use the index only to narrow candidates, then recheck created + ttl before selecting and deleting rows. Calendar intervals depend on the session time zone, so making the UTC key authoritative would change DST and month-end behavior. Reads and CAS keep their existing predicates; physical deletion can lag without changing logical expiry.
Provision automatically only on empty supported tables with pooled clients. Check emptiness before taking a write-blocking lock, then recheck under READ COMMITTED with short timeouts to avoid building against a stale snapshot. Skip tables with row-level security because policies could hide existing rows from the emptiness check. If setup fails, the store remains usable without the index.
Populated tables and initialized: true deployments use CREATE INDEX CONCURRENTLY followed by ANALYZE, as documented in docs/manual/kv.md. This keeps migration under operator control and gives the planner expression statistics. Index maintenance adds write/storage costs, and expiration timestamps outside PostgreSQL's range now fail writes atomically, including from older Fedify writers.
Store UTC-calendar expiry keys in a partial expression index while
retaining the original session-calendar TTL checks. Narrow bounded
sweep candidates without changing reads, CAS, or housekeeping ownership.
Provision indexes only on empty tables through a bounded private
transaction. Document concurrent migration for populated and externally
managed tables, rolling upgrades, calendar skew, and timestamp limits.
Cover old writers, TTL transitions, expired-key CAS, identifier transforms,
concurrency, RLS, non-owner access, and partitioned or inherited layouts.
Verify actual custom and generic sweep plans over 80,000 live rows.

Walkthrough
PostgresKvStore now uses a UTC expiry expression index to find cleanup candidates while retaining the existing session-calendar TTL check. Initialization can provision the index for eligible empty tables. The change adds tests and migration guidance for existing tables, rolling upgrades, and timestamp-range limits.
The cleanup query uses a UTC expiry expression as an index search key and retains the session-calendar TTL predicate. Tests cover query plans, TTL operations, non-UTC calendar behavior, and timestamp overflow.
Conditional index provisioning packages/postgres/src/kv.ts, packages/postgres/src/kv.index.test.ts
Initialization checks for a matching index and provisions one for eligible empty tables. Tests cover concurrent initialization, table layouts, existing or conflicting indexes, and restricted access.
Migration and rollout guidance docs/manual/kv.md, packages/postgres/README.md, claude-plugin/skills/fedify/SKILL.md, changes.d/postgres/indexed-expiry.md, CHANGES.md
Documentation describes automatic and manual index setup, rollout compatibility, TTL behavior, and timestamp-range limitations.

Explanation
Issue #1309 requires indexed expiry lookup, preserved TTL and CAS semantics, bounded cleanup, an upgrade path, rolling compatibility, and validation. packages/postgres/src/kv.ts adds a UTC expression-index candidate predicate, then rechecks created + ttl before selection and deletion. It preserves the bounded sweep and existing read and CAS predicates. The initialization logic provisions the index only for eligible empty tables and safely skips or logs unsupported cases. docs/manual/kv.md documents concurrent migration, ANALYZE, custom identifiers, initialized: true, inheritance, partitions, overflow, and rolling upgrades. packages/postgres/src/kv.index.test.ts covers fresh and legacy tables, TTL transitions, expired-key CAS, old writers, transformed identifiers, provisioning conflicts, and custom and generic sparse query plans. The expression index satisfies the issue's indexed-lookup requirement without requiring a new column.

an invalid index, and explain when each identifier needs quoting.
Make the recovery order explicit: rebuild the index, then ANALYZE.

Reject UNIQUE and multi-key indexes in the expiry-index matcher.
The matcher checks only the first key, so a matching UNIQUE or multi-key index can make initialization skip the intended single-key non-unique index. The UNIQUE index can then reject valid writes that produce duplicate expiry values.
Add catalog checks for the intended index shape:

Accept only non-unique single-attribute indexes during expiry index
provisioning. A matching first key on a wider index does not establish
that cleanup has the intended dedicated index.
Preserve operator-owned indexes and constraints. Cover UNIQUE and
multi-key indexes, including custom and generic sweep plans over
80,000 rows, and document the stricter matching criteria.
fedify-dev#1334 (review)
Changelog: none
Assisted-by: Codex:gpt-6.1-sol
Addressed the expiry-index matcher finding in c2948a7. The matcher now requires a non-unique, single-attribute index. On empty tables, regression tests confirm that existing UNIQUE/multi-key indexes are preserved and a dedicated index is added; custom/generic sweep plans over 80,000 rows avoid a sequential scan.
Adding the non-unique index cannot lift an existing UNIQUE constraint, which still applies to writes. Provisioning does not drop or alter operator-owned indexes.
