Fix Medicaid payer name in LangChain RAG Cypher prompt - #914

The cypher_generation_template listed 'Medicare' as a payer, but payers.csv only contains 'Medicaid'. When the LLM used the listed name, the Cypher query matched no payer and the chain answered "I don't know the answer." Reported by a reader on the tutorial.
