Removes the accessibilityExpert tool. Nobody uses it, so it goes now rather than after a deprecation release.

What changes

  • Tool: accessibilityExpert is no longer registered. Its handler in src/tools/accessibility.ts is removed.
  • Knowledge-base client: src/tools/accessiblity-utils/accessibility-rag.ts is deleted. It called accessibility.browserstack.com/api/tcg-proxy/search, and nothing else used it.
  • Tests:
    • the tool's response test is removed
    • the manual prompt-injection case for the knowledge-base client is removed
    • the tool-registration test now checks for the remaining four tools and that accessibilityExpert is gone
  • Docs: its row is removed from docs/TOOLS.md.

The remaining accessibility tools are unchanged: startAccessibilityScan, createAccessibilityAuthConfig, getAccessibilityAuthConfig and fetchAccessibilityIssues.

This PR first marked the tool deprecated (0be9a14), then removed it outright (e8b8ac9). The net diff against main is the removal only.

Verification

  • The registration test failed before the removal, while accessibilityExpert was still registered, and passes after.
  • No test is left referencing the tool or its client.
  • npm run build passes on Node 22: lint, prettier, 1349 tests, tsc.
  • Against main's 1352 tests, the 3 that go are the two removed tests and the per-tool description-length check for accessibilityExpert.

accessibilityExpert is going away. Until it is removed it keeps answering
from BrowserStack's accessibility knowledge base, so nothing that calls it
breaks, but its description and title now say it is deprecated, each answer
starts with a note that it will be removed, and the tool list in
docs/TOOLS.md marks it.

Nobody uses it, so it goes now rather than after a deprecation release:
the tool, its knowledge-base client (accessibility-rag.ts), its tests,
the manual injection case for it, and its row in docs/TOOLS.md. The
accessibility tools are now startAccessibilityScan,
createAccessibilityAuthConfig, getAccessibilityAuthConfig and
fetchAccessibilityIssues.

Telemetry shows the tool in use (about 700 calls from about 120 users in 30 days), so it
is not removed here. If it is to go, it gets a deprecation first, with the replacement
named, and a removal in a later release.

This reverts merge commit 07ecdda, which brought #512 into #502.
